At WWDC in June, Apple delayed the health feature everyone expected. Project Mulberry, the AI coach, and the Apple Health+ subscription that was supposed to carry it, stayed off the keynote so Apple could rebuild the Watch’s heart-rate engine first. Three months later, on 9 September, it started interpreting that data: a 0–10 readiness score, a Health Age, an Insights tab, and a Quest lab panel inside the Health app [1][2].
What it still did not ship is a paid coach, or a developer API. That combination — interpretation as a consumer feature, gated to new hardware, free, and walled — is the long-term story of the event. Not the keynote slides.
What did Apple actually ship for health on September 9?
Short answer: sensing and readiness on 14 September on the newest watches; interpretation later this year in the Health app; nothing new as a HealthKit contract. The useful way to read the announcement is not the feature list. It is the gate next to each feature [1][5][6].

Read down the right-hand column and the pattern is the same one we mapped in the iOS 27 beta playbook: Apple made its own health experience richer, on its own devices, on its own schedule. GymKit on iPhone, Workout Buddy without a nearby iPhone, and better treadmill distance estimates also landed; they are incremental, and that post already covers the catch on each [1]. The September event is not those features. It is Apple joining the interpretation business.
Is Apple’s readiness score a WHOOP killer — or a category?
Short answer: it is Apple turning a specialist product into a Watch default. That legitimizes the category. It does not hand you the score.
WHOOP and Oura have sold a daily capacity number for years. Apple’s version is a 0–10 score from recent activity, vitals, and sleep score, with four recommendations — Recover, Pace Yourself, Ready, or Go For It — that update during the day as workouts and daytime vitals come in [1]. Apple has not said which integers sit in which bucket. The algorithm was developed with data from the Apple Heart and Movement Study [1]. That is a serious construction claim. It is not independent validation of the composite against an external reference. How that 0–10 is constructed versus Oura’s nine contributors, WHOOP Recovery, and Garmin’s two different numbers is the readiness-score map.
For product teams, three consequences sit on top of the number itself.
Consumers will expect the shape. A single daily score with a plain-language verb is now what the newest Apple Watch is. Matching that language on Apple-only, for the subset of users who just bought a Series 12, is not a strategy. It is table stakes on one SKU.
Apple’s score is not yours. It is 0–10, Watch-new-SKU-only, and not announced as a HealthKit type. You cannot query it, you cannot re-weight it, and you cannot show it to an Android user or to the person whose Watch is a Series 10. A readiness signal that degrades to phone data when no wearable is present is a different product problem than decorating Apple’s 0–10.
Category legitimization cuts both ways. Apple just told the market that daily readiness is a default, not a niche. That is good for anyone already selling the idea. It is bad for anyone whose only differentiator was having a number WHOOP had and Apple did not.
Why does the richest health data still require a new watch?
Short answer: because the sensing upgrade is how Apple sells Series 12 and Ultra 4. Health is the attach, not the coverage strategy.
The Health Sensing System is the actual hardware story. Heart rate is measured every five seconds, all day. HRV is measured as often as every five minutes — Apple’s figure is 24 times more frequently — and split into Recovery HRV for daily stress versus overall HRV for longer-range cardiovascular context [1][2]. Overnight Vitals now include Recovery HRV against a personal baseline, and a daytime view can surface shifts earlier [1]. Apple published a study of more than 1,000 participants claiming the highest heart-rate accuracy among leading wearables [3]. Hypertension notifications are not the September headline: they are a 2025 optical-pattern feature on Series 9 and later, not a cuff blood-pressure reading.
All of that is real, and none of it comes to the installed base. watchOS 27 installs on Series 9 and later, SE 3, and Ultra 2 and later [1]. Readiness, five-second heart rate, and five-minute HRV do not. Health Age requires a Watch, full stop [1]. Most adults still do not wear a Watch — smartphone ownership runs far ahead of wearables, and the units that produce rich health data are concentrating in the premium half. Everyone outside that newest-Watch slice gets a prettier Health app later this year, not the new sensing system. We have already documented what Apple Health looks like without an Apple Watch. September sharpened that picture.
The long-term take is not that Apple’s heart rate got better. It is that Apple uses health to refresh the Watch. Denser HRV is a reason to buy Series 12. It is not a reason to believe Apple will cover the users who will never wear one. Products that assume “Apple now does readiness” are assuming a 2026-watch feature for a user base that mostly does not have one.
For developers, the denser stream is another overlapping source. Series 12 heart rate writing alongside phone motion, GymKit, and a third-party wearable is exactly the deduplication problem that gets worse every time Apple ships a better sensor — unless you treat the new samples as an optional input behind a normalization layer, which is why fitness products already read one activity stream instead of three overlapping ones.
Did Apple launch an AI health coach?
Short answer: no. Mulberry arrived as Insights, inside the free Health app, later this year, with no Health+ SKU.
The Insights tab surfaces a summary that updates through the day across heart, sleep, readiness, fitness, vitals, and cycle tracking, plus a For You section of personalized recommendations and articles [1][4][5]. That is the coach, scaled back and unnamed. Reporting before the event was explicit: the comprehensive Mulberry service and a standalone Health+ offering were folded into the ordinary Health app rather than launched as a subscription [7]. September confirmed the fold. It did not confirm the SKU.
That is a pricing decision, not a technical one. Two days before the event we mapped what consumers actually pay for health insight: Oura at $5.99, Garmin Connect+ at $6.99, Google Health Premium — the Gemini coach — at $9.99. Apple’s Health app was on that map at $0, and it still is. Google shipped the paid coach in May. Apple had the distribution, the sensor, and a three-month window after WWDC, and it still chose not to convert health into a recurring product.
That is a choice to sell watches and iPhones, not a health SKU. It is why the AI coach race did not end on 9 September. Google charges for interpretation. Apple is giving a thinner version away, later, in US English, on Apple Intelligence hardware. The question was who would get paid to reason over whose record. Apple declined the payment. It did not open the record.
What do Quest labs inside the Health app change?
Short answer: Apple can now hold daily PPG and yearly A1c in the same place. That is the fused record everyone else has been assembling. Third-party apps still cannot query the fusion.
Users will be able to buy a panel of more than 50 biomarkers for $119 at about 2,000 Quest Diagnostics locations in the US, and fold those results into the Longevity view alongside Watch, iPhone, AirPods, clinical records, and third-party apps [1][4]. Oura already sells Health Panels through Quest at $99. WHOOP already sells Advanced Labs. The consumer blood-testing boom was underway before Apple walked into it with the default Health app as the storefront.
The strategic shape is the one we traced in what builds a moat in health data: a daily optical sensor is one modality; a blood panel is another; the company that sits on both owns a record nobody can backfill. Labs were the missing modality. $119 is Apple completing the stack.
What it is not: a lab API for your product, a Health Age endpoint, or a fused view you can read. Apple will interpret A1c next to VO2 max inside Health. Your app still sees whatever HealthKit already exposed, plus whatever you collect yourself. The continuous-plus-episodic loop closed for Apple. It did not close for anyone building on Apple.
Is Health Age still a product?
Short answer: not as a differentiator on iOS. Biological age just became default UX on the newest Watch.
The Longevity tab scores heart health, sleep, mental wellbeing, movement health, metabolic health, hearing, and nutrition, and Health Age sits inside it — VO2 max, resting heart rate, sleep, HRV, plus optional A1c and LDL — showing how those metrics track against chronological age [1][4]. Movement evaluations add an on-device, no-video-stored assessment of flexibility, strength, balance, mechanics, and VO2 max, using the iPhone camera and Watch [1].
Wearable biological age was a specialist category in March. In September it is a tab in Apple Health. The PM implication is blunt: shipping “we have a bio-age number” is no longer a reason a user stays in your iOS app. The remaining differentiation is whether the number works without a Watch, whether the factors are explainable, and whether anyone has validated the composite rather than citing the components. Apple said the algorithm is “informed by the best available science” [1]. That is evidence-based construction. Treat it that way.
What does perimenopause in Cycle Tracking change?
Short answer: Apple is making life stage a first-class Health dimension. Sex-blind recovery scores will look dated next to the OS.
This is not a September surprise. Peri and menopause support was already in the iOS 27 betas, and we covered the builder catch then: iOS-only, dependent on logged cycles, aimed at users 40 and over for deviation alerts [5]. The event made it official. Users can log perimenopause or menopause, track symptoms, and get a prompt to speak to a doctor if they log bleeding after menopause. Users 40 and over can get a notification when logged cycle patterns look like perimenopause [1].
The durable take is the one in women’s health data: resting heart rate, HRV, and temperature shift across the cycle in ways most scoring models still ignore. Apple is now treating the menopausal transition as in-bounds for the platform. Products that still compute recovery as if every user were a 30-year-old man are not competing with WHOOP. They are competing with the Health app on the user’s home screen.
What didn’t move for builders?
Short answer: HealthKit still measures, still does not interpret for you, still does not span Android, and still does not cover the phone-only majority. Apple’s new scores live in Apple’s UI.
The foundation we keep returning to in HealthKit versus Health Connect did not move. There is no announced readiness type, no Health Age type, no Insights feed. Confirm what actually writes before anyone on the roadmap treats a keynote screenshot as a data source — the same discipline as camera nutrition, which still does not sync the headline food-photo read into Health.
The builder posture is triage, not panic and not imitation.
- Take the denser heart-rate and HRV stream as an optional upgrade for Series 12 and Ultra 4 users. Let it flow through whatever already turns raw samples into scores. Do not wait for Apple to expose the 0–10.
- Do not feature-match Health Age and Insights on Apple-only. Those are consumer expectations on the newest Watch, not a TAM. Design for mixed hardware, Android, and the people who will never wear a Watch.
- Do not assume Apple will stay out of scoring, longevity, or bloodwork. It is in all three. The durable gap is not that Apple cannot interpret. It is that whatever it interprets, it interprets inside its walls — never as a neutral layer across iOS, Android, and the phone.
When Apple’s heart-rate engine improves, the signal should flow straight through. When Apple ships a score, that score should not become the product’s load-bearing input. A Cupertino release is upside you capture, not a foundation you inherit. That is the problem a cross-platform data layer is for, and it is the problem we work on at Sahha.
