7 Best Terra API Alternatives in 2026
Terra is the broadest direct-to-cloud wearable aggregator in the category, and the right answer for many teams. The alternatives below matter when you need scoring included rather than billed as an add-on, a way to evaluate without paying up front, behavioural signal rather than raw metrics, or data residency Terra does not currently offer.

What the $399 plan does not include
Terra Quick Start lists at $399/mo billed annually and includes 100,000 credits, which covers roughly 500 active authentications at their published rate of 200 credits each. Health Scores are a separate add-on from $499/mo, so a team that needs scoring is at $898/mo before writing any code. There is no free tier; the entry plan carries a 30-day money-back guarantee instead.
Every Terra score requires HRV, resting heart rate, respiratory rate, SpO2 or sleep stages. Terra reads Apple Health and Health Connect through its Mobile SDK, so phone data arrives either way, but those scores return null without wearable-grade inputs.
Neither Terra nor Sahha publishes a validation study for the scores it ships. Sahha publishes the research underneath its markers with sample, ethics approval and limitations stated. Terra describes its scores as validated without naming the study behind them.

Sahha
our productBest for
Covering every user, not just the ones who own a wearable.
The phone-first option in this list. It reads the same OS health stores as everyone else here, and differs in scoring from them when no wearable is connected.
- Pricing
- 30-day free trial, then Starter at $299/mo billed annually for 1,000 users. Every feature, including all scores, biomarkers and archetypes, is included on every plan.
- The catch
- Direct cloud connections cover 4 providers including Garmin, Oura, and WHOOP. Everything else arrives through an SDK inside your app, so a product spread across many wearable brands with no app to ship will find Terra’s roughly 88 direct integrations a better fit.
- Founded 2019 · Spring, Texas
Best for
Healthcare-grade delivery and FHIR.
The closest structural analogue to Terra, with 8 direct cloud APIs plus device-level SDKs for Apple Health, Health Connect and Samsung Health.
“Solve wearable integration problems with ROOK’s API.”
ROOK in their own words · their homepage - Pricing
- Core $399/mo for 750 active users. No free tier.
- The catch
- Scoring costs extra. ROOKScore is a $249/mo add-on on Core, granular data another $249/mo, and notification webhooks another $99/mo. Historical backfill is 7 days for API sources.
-
Best for
Lab testing and wearables under one contract.
Wearable aggregation plus a nationwide US lab-ordering network. Computes its own sleep, recovery, stress, strain and chronotype scores against a 21-day rolling baseline.
“Power Every Diagnostic Touchpoint at Scale.”
Junction (formerly Vital) in their own words · their homepage - Pricing
- No pricing is published on junction.com. The Vital-branded pricing page that Junction's current docs still link to lists $0.50/user/mo with a $300 minimum.
- The catch
- Junction now leads with diagnostics. Its homepage mentions labs 24 times and wearables twice, and there is no product-level nav item for device connectivity.
- Founded 2016 · Berlin, Germany
Best for
EU data residency and European regulatory review.
Berlin-based, developed and hosted entirely in Europe, with an analytics layer that outputs sleep-derived health risk assessments in actuarial terms.
“Transform Healthcare with Wearable API and Health Data Analytics.”
Thryve in their own words · their homepage - Pricing
- Free trial at 25 users. Start Up from EUR 499/mo for 500 users. Self-serve via Stripe.
- The catch
- Historical backfill defaults to 14 days and is gated to higher tiers. No validation study is published for its risk models, and no ISO certificate is downloadable despite the certification claim.
- Founded 2025
Best for
Self-hosting, full data ownership and auditable scoring.
MIT-licensed, self-hosted, and free. Roughly 2,200 GitHub stars and 52 contributors, with releases every two to four weeks since December 2025.
“Health intelligence, not just health data.”
Open Wearables in their own words · their homepage - Pricing
- Free under MIT. Enterprise support, BAA and HIPAA-eligible deployment are quoted per project.
- The catch
- Their own pricing page describes the scoring as "basic health scoring algorithms (sleep, resilience)", and their docs mark the Apple Health mobile SDK as "coming soon". Compliance is the deployer's responsibility, and they state they are not HIPAA-certified as a product.
- Founded 2022
Best for
Nutrition photo analysis and lab parsing in one API.
Around 23 direct cloud OAuth providers plus its own mobile SDK, with nutrition photo analysis and lab report OCR alongside wearable data.
“360 degree health data for AI.”
Spike in their own words · their homepage - Pricing
- From $450/mo. No free tier and no self-serve signup.
- The catch
- Spike Technologies was acquired by Raintree Systems in July 2026, and the acquisition materials describe an agentic voice AI product for revenue cycle management without mentioning the wearables API. Its scores are gated to Tier 2 and above, and three of the four have no published methodology.
-
Best for
Academic and clinical studies with no engineering team.
A turnkey research platform rather than a developer API, with participant apps, surveys, compliance tracking and REDCap-ready exports.
- Pricing
- From EUR 50/mo, which covers 5 device licences. Additional devices are EUR 1 to 7.50 each per month, and API access is a further 20% on the licence cost.
- The catch
- No SDK, no webhooks, and no FHIR. API access is sales-gated and costs extra, and their own documentation instructs developers not to request data at millisecond level.

Terra alternatives, answered
Which Terra alternative has the most device integrations?
Terra itself, on the numbers each vendor publishes. Its documentation enumerates roughly 88 direct server-to-server connections, more than any alternative on this page reaches directly. If breadth of direct cloud coverage is your deciding factor, the honest answer is to stay.
What is the cheapest Terra alternative?
It depends whether you need scoring. Terra Quick Start is $399/mo billed annually and Health Scores are an add-on from $499/mo, so a team that needs scoring is comparing $898/mo. Sahha Starter is $299/mo for 1,000 users with every score, biomarker and archetype included. Open Wearables has no licence fee at all, if you can run it yourself.
Is there a free Terra alternative?
Open Wearables is MIT-licensed with no per-user fee, but you host and operate it, so the cost moves from a subscription to engineering time and on-call. Sahha is free for 30 days with 25 development users, then $299/mo. Terra has no free plan; its entry tier carries a 30-day money-back guarantee instead.
Do any Terra alternatives work without a wearable?
Sahha scores sleep, activity and mental wellbeing from phone signal alone. Terra reads Apple Health and Health Connect too, so it ingests phone data either way, but its documentation states no score computes from step count alone and that a score will not be generated or may return as null without its required inputs.
What is actually hard about migrating off Terra?
Re-consent. Provider authorizations cannot transfer between vendors, so every user who connected a wearable authorizes again wherever you go. No tooling on either side changes that, and the cost is identical in any direction, including away from Sahha.
Can I run Terra and an alternative at the same time?
Yes, and it is usually the cheaper path if a contract still has months to run. Point new signups at the new provider while existing connections keep running on Terra, and let attrition move the rest across.
